The death of a doctor named Aulia Risma<\/a>\u00a0, who was studying for the Specialist Doctor Education Program (PPDS), has reopened the dark veil of education that occurs in the medical community.<\/p>\n

Moreover, what happened to Aulia Risma is not the first time it has happened.\u00a0In 2020<\/a>\u00a0, a PPDS student also ended his life with strong suspicions of\u00a0bullying<\/em>\u00a0.<\/p>\n

In fact, Indonesia is still experiencing\u00a0a shortage of doctors<\/a>\u00a0, especially specialist doctors, in various regions outside Java.<\/p>\n

The existence of bullying<\/em>\u00a0practices\u00a0is an irony in the midst of the shortage of doctors. This shows that there is something wrong with the education system, especially the PPDS education system, and emphasizes the importance of a humanist approach in the education of specialist doctors.<\/p>\n

The effects of seniority and systems<\/strong><\/h4>\n

Specialist medical schools have\u00a0a high prevalence\u00a0<\/span>of bullying , not only in Indonesia\u00a0<\/span><\/em>but also throughout the world<\/span><\/a>\u00a0. In fact,\u00a0<\/span>a study in Sweden<\/span><\/a>\u00a0showed that in addition to\u00a0<\/span>bullying<\/span><\/em>\u00a0, female PPDS students also often experience\u00a0<\/span>sexual violence<\/span><\/a>\u00a0. This means that they have the potential to experience at least two traumatic events during their studies.<\/span><\/p>\n

According to data from the Ministry of Health (Kemenkes), there have been more than a thousand complaints of bullying or alleged bullying with around\u00a0<\/span>30% of the reports verified<\/span><\/a>\u00a0as\u00a0<\/span>bullying<\/span><\/em>\u00a0cases . Referring to the data,\u00a0<\/span>bullying<\/span><\/em>\u00a0practices are common.<\/span><\/p>\n

Bullying<\/span><\/em>\u00a0practices\u00a0are seen from how\u00a0<\/span>junior students<\/span><\/a>\u00a0often receive insults and curses from seniors, even for personal orders, such as picking up laundry from the laundry, picking up senior children from school, and\u00a0<\/span>paying vehicle taxes<\/span><\/a>\u00a0. In addition, there are also practices that lead to physical and mental violence\u00a0<\/span>, and also extortion<\/span><\/a>\u00a0against PPDS students.<\/span><\/p>\n

This bullying that leads to extortion creates\u00a0<\/span>another financial burden<\/span><\/a>\u00a0for students, outside of the official fees set by educational institutions. If this burden is not met, it will impact the continuity of PPDS students’ studies.<\/span><\/p>\n

In addition to having to finance the personal needs of seniors and consultants (specialist doctors or subspecialists), PPDS students also face heavy study pressure and workloads so that they can also experience bullying caused by the system.<\/span><\/p>\n

For example, they have to work as much as 80 hours per week without pay\u00a0<\/span>because they perform tasks related to the duties and functions of a doctor such as <\/span>a coder<\/span><\/em> , porter,<\/span><\/a>\u00a0etc.<\/span><\/p>\n

Humanistic education as a solution<\/strong><\/h4>\n

Research shows<\/span><\/a>\u00a0that the practice of humanistic education in medical education has an impact on the quality of future medical and health jobs. This is because humanistic education emphasizes\u00a0<\/span>compassion, sensitivity, sensitivity and dialogue<\/span><\/a>\u00a0when dealing with patients.<\/span><\/p>\n

Bullying<\/span><\/a>\u00a0that occurs in the medical education environment, especially PPDS, is very contrary to the meaning and purpose of education. This is because bullying not only affects the victim\u2014who has the potential to experience a decline in physical and\u00a0<\/span>mental<\/span><\/a>\u00a0health \u2014but also reduces the quality of service and patient safety.<\/span><\/p>\n

Involve all parties<\/strong><\/h4>\n

Many studies have been published on bullying in medical education environments and efforts to address it.\u00a0<\/span>Studies in 2019<\/span><\/a>\u00a0and\u00a0<\/span>2022<\/span><\/a>\u00a0revealed that bullying in medical education is a violation of basic medical ethics and a violation of human rights, thus requiring institutional policy intervention.<\/span><\/p>\n

Currently, the police have begun investigating the Aulia Risma bullying case. While waiting for\u00a0<\/span>the results of the investigation<\/span><\/a>\u00a0, for example, the government should move quickly to conduct a thorough investigation at the medical faculty and the hospital that is the organizer of the education. This is to ensure\u00a0<\/span>zero bullying<\/span><\/em>\u00a0in medical education in Indonesia.<\/span><\/p>\n

So far,\u00a0<\/span>the Minister of Health’s Instruction<\/span><\/a>\u00a0Number HK.02.01\/MENKES\/1512\/2023 concerning the prevention and handling of bullying against students in teaching hospitals within the Ministry of Health, has not provided a deterrent effect on perpetrators of bullying, especially in PPDS.<\/span><\/p>\n

The government also needs to form a special anti- bullying<\/span><\/em>\u00a0task force\u00a0in the medical education environment. This task force consists of various elements, not only from within the university, hospitals, the Ministry of Health, the Ministry of Education, Culture, Research, and Technology (Kemendikbudristek), the police, but also several independent experts, such as psychologists, sociologists, and legal experts.<\/span><\/p>\n

This task force is tasked with identifying signs of\u00a0<\/span>bullying<\/span><\/em>\u00a0, accompanying victims, and campaigning for anti-\u00a0<\/span>bullying<\/span><\/em>\u00a0movements that are integrated into the learning curriculum, both formal and informal (extracurricular activities).<\/span><\/p>\n

As an initial step, campuses can optimize the Sexual Violence Prevention and Handling Task Force (PPKS) to be the vanguard in preventing various acts of violence that occur on campus, including bullying.<\/span><\/p>\n

This bullying<\/span><\/em>\u00a0prevention solution\u00a0also needs to involve the National Human Rights Commission (Komnas HAM) because\u00a0<\/span>bullying<\/span><\/em>\u00a0has robbed human rights. One of them is by using\u00a0<\/span>social media<\/span><\/a>\u00a0as an alternative to the\u00a0<\/span>bullying<\/span><\/em>\u00a0complaint post in the medical education environment. Because, it is possible that\u00a0<\/span>bullying<\/span><\/em>\u00a0victims feel unsafe or uncomfortable to report through\u00a0<\/span>the Ministry of Health post<\/span><\/a>\u00a0.<\/span><\/p>\n

From a personal perspective, PPDS participants need moral support from family and close friends to reveal the bullying cases they experience, for example through popular <\/a><\/span>podcast<\/span><\/em><\/a>\u00a0channels\u00a0. Family and close friends play a role in accompanying PPDS participants who are victims\u00a0<\/span>of bullying<\/span><\/em>\u00a0so that they do not feel alone in facing this problem.<\/span><\/p>\n

No less important, eliminating\u00a0<\/span>bullying<\/span><\/em>\u00a0requires a commitment from every leader, educator, employee, and student in the PPDS environment to work together to break the chain of\u00a0<\/span>bullying<\/span><\/em>\u00a0that\u00a0<\/span>has been deeply rooted<\/span><\/a>\u00a0for decades.<\/span><\/p>\n

One thing that must be eliminated is the claim that\u00a0<\/span>bullying is part of mental development<\/span><\/a>\u00a0. This view perpetuates the dominance of seniority influence (power) or\u00a0<\/span>in Gramsci’s language called hegemony<\/span><\/a>\u00a0, namely how\u00a0<\/span>bullying<\/span><\/em>\u00a0in PPDS is produced and reproduced by leading resident doctors as juniors to act according to the wishes of seniors\u2014as if without coercion or violence.<\/span><\/p>\n

In fact, the hegemony of mental development like this harms humanity and will only produce doctors\u00a0<\/span>with a feudalistic mentality<\/span><\/a>\u00a0.<\/span><\/p>\n

It is time for every leader, both in educational environments and hospitals, educators, employees, and students in the PPDS environment to realize, acknowledge, and improve their education system, so that it becomes more humanistic and treats doctors humanely.<\/span><\/p>\n
